The legal authority for local health departments is found primarily in the Ohio Revised Code, while specific implementation guidelines are found in the Ohio Administrative Code and Shelby Charter Section 50.

NURSING SERVICES
The Shelby Health Department contracts with Shelby Home and Public Health to provide nursing services.  The Shelby Home and Public Health office is located at 142 N. Gamble Street, Shelby, OH  44875.  The Phone number is 419-342-6366. Services include:

VITAL STATS- Birth and Death Certificates

The Vital Statistics division of the Shelby Health Department maintains records of all births in the City of Shelby since 1878.  Death Certificates are available from 1908. City of Shelby births and deaths before those dates are kept at the Richland County Probate Courthouse in Mansfield, OH.

Births and deaths occurring in other cities, counties and states must be obtained from those cities, counties and states.  If you do not know the location of an Ohio birth or death, you may obtain those certificates from the Ohio Department of Health in Columbus.  All original Ohio birth and death certificates are permanently placed on file at the Ohio Department of Health in Columbus. Certified copies of certificates are available at Shelby City Hall.  Certified copies cost $25.00.  Certificates may be obtained through our walk-in service or by mail and no appointment is necessary.  We accept cash or checks.  All questions regarding birth and death certificate copies or spelling corrections should be directed to the Registrar at 419-347-5131. Another service provided by the Vital Statistics division is acknowledgement of paternity affidavits.  The completion of paternity affidavits is voluntary and acknowledges that a parent and child relationship exists.  The affidavit places responsibility for support of the child on the alleged father.

The Vital Statistics Department also issues burial-transit permits to area funeral directors.
