Temporary Food Application and Information

Instructions for Obtaining A Temporary Food Service Operation License

 

Fill out the Facility Layout and Letter of Intent after you read through the Operating Standards Checklist and the Equipment/Supply Checklist handouts.  Return the Facility Layout and Letter of Intent, but keep the handoutsYou must follow the requirements on the handouts.  If you feel that something does not apply to you or you want to do something different, you must get prior approval.  Also, fill out and return the Temporary Food Service Application along with the $35 fee at least one week before the event.

After we receive the application, fee and letter of intent the health department will prepare a temporary food service operation license to issue to you the day of the event.  An inspection of the operation will be made to insure compliance with the updated Ohio Uniform Food Safety Code the day of the event and the license will be issued if everything is found to be satisfactory.
